If the denial relies on a wrong date, address, ownership fact, or description of the loss, respond with records that show the correction. Keep the denial and your response in the claim file.
Compare the letter with the complete policy
Read the definition, coverage grant, exclusion, condition, and endorsement together. A single quoted sentence can be misleading without the surrounding form.
Read the denial line by line
A denial letter should identify why the insurer says coverage does not apply. Separate the stated reason from your disagreement with the result.
- What policy provision is quoted?
- Is the dispute about cause of loss, exclusion, ownership, documentation, timing, or amount?
- Did the insurer request information it says was not provided?
